  1. Submit the form below.​

  2. Make your donation using the "Make Your Scholarship Donation" button below.​

To complete the form, please click the "Submit" button below, and scroll down to the "Last Step" section to make your donation.

LAST STEP:

After completing the form above, please use the button below to make your scholarship donation. The button will take you to the institute's donation page on the University of Cincinnati Foundation's website.

 

On that page,

  1. Select the red "Other" button under the "Donation Amounts" section, and enter "$1,000."

  2. Skip the "Type of Gift" and "Tribute Information" sections, scroll down to the bottom of the page, click the red "Continue" button to complete the payment process.
